Updated the "tx_smtp" section, added notes about:

* the 'in transit' flag and how to ignore or fix the problem
* the 'usual' DNS server IP address
* detecting outbound SMTP filtering/blocking
* avoiding a relay "loop"
* detecting and correcting SMTP relay authentication problems
