Restructured the Appendix TOC after the removal of Appendix D and updated links to the proper bookmarks

The other tabs and dialogs should be added too. :-(

Removed DOVE-Net configuration information (pre-configured in v3 and available elsewhere).

(really should be moved to AFTER the sections explaining how to add file areas and import file descriptions).

Removed Appended D (all the contact information was outdated).

Removed NN: prompts (not hardcoded).

Okay, reverted back to "../images". Apparently, if you don't "open a web" in FP, it doesn't do includes right. <shrug>

Put file dumps back in (removing ex-ascii chars).

Using smb.html as the reference doc, the appears in FP (with the "../images" path), but not in a web browser. <shrug>
Changing to "images/" fixes the logo in the browser, but now the logo doesn't appear when viewed with FP.
