Using strdup() instead of malloc()/strcpy().

Default content-type is "text/plain". The same should probably done for
Unix-CGI (?).

New ability to choose which subs are displayed while reading messages.  This sets the New Scan On/Off/To You on the Telnet side as well.  (And related updates to existing files.)

Moved 301 and 302 error strings to shared string constants.

Disabled (defaults to false) and LogRequest (defaults to true), which along
with LogResults will log the entire http_request object for detailed logging
to data/formmail.log for security or debugging purposes.

The default referes array is empty and recipients array allows mail to 'sysop'
only. In order to over-ride these values, the ctrl/formmail.ini must be created
or edited.

Optionally append results to data/formmail.log.
